Inclusionary Zoning Orientation Form (IZO)

1. If you live in Washington, DC have you completed the Inclusionary Zoning Orientation?

*If your household has not yet taken the orientation OR if you’ve not taken it in the last 2 years, you will not be added to the waitlist. 3. Are ANY household members currently Full or Part-Time students?
5. Are any residents college students?
6. Will anyone be a college student within 6 months?

*If ANY household members are full-time students, the household will not be eligible unless you are a dependent of Parents, Guardians or the resident of whose household would otherwise meet the requirements for the Inclusionary Zoning Program.

*Adults is defined as anyone at the legal age of 18 or older. If the individual will be 18 within 6 months of residency, please notify the staff on this application.

8 (b). Will any individal who will be residing in the home at least 6 months BEFORE turning 18 years of age?

* If the individual will be 18 within 6 months of residency, notify the staff here.

*Child/Children is defined as anyone under the legal age of 18. In many communities, children 18 months and younger are not considered part of the total household size when determining the apartment size at move-in

*Enter total GROSS annual income for all household members. Include income from employment, SSA/SSI/SSDI, TANF, Child Support (court ordered or otherwise), Alimony, Retirement, Pension, Unemployment, Military Pay, and Recurring Gift Income. Include net income for self-employment. Please review income guidelines to confirm you are eligible for the 80% AMI IZ program.

*Assets and income from assets are part of the total household income calculation. Values will be determined when completing the initial certification.

13. Will you use and make Eden Estates your primary home residence and maintain it according to all inspections, rules and regulations?

*All must use the property as their primary residence.

14. Does anyone in your household need reasonable accommodation due to a disability that requires an additional bedroom and/or are there any accessibility features needed for the household? If yes, it is required to be listed here.

*Verification from a qualified third-party will be required

16. Does any household member own residential real estate?

*Real estate ownership may impact overall household eligibility depending on the specific affordable program for which you are applying. Additional verification of real estate ownership will be required at the time of an apartment offer. If information intentionally left off legal ramifications may apply.

17. Does any member of the household LIVE in Washington DC?
18. Does any member of the household WORK in Washington DC?
